To receive replacement medals, awards, and decorations you must contact NPRC (National Personnel Records Center). This organization is part of the National Archives and Records Administration (NARA).
Begin by downloading a copy of the Standard Form SF-180, available on our Forms Website: http://www.tpromo2.com/usvi/forms/standard-form-180.pdf. (Note: This is a PDF file. Fill out the proper portions of the form and submit to the proper department:

When requesting medals, include the following information:
NPRC prefers that the above request be submitted via an SF180 form, "Request Pertaining to Military Records," available on the U.S. Veteran Information (non-governmental) web site.
