How Do I Apply For A Headstone or Marker For A U.S. Veteran?

According to the Federal Benefits for Veterans and Dependents, 2005 Edition, you must take the following steps:

  1. Complete VA Form 40-1330, Application for Standard Government Headstones or marker for Installation in a Private or State Veterans Cemetary
  2. Include a copy of the veteran's military service discharge document
  3. Send both to the following:

    Memorial Programs Service (402E)
    Department of Veterans Affairs
    810 Vermont Avenue, N.W.
    Washington, D.C. 20420-0001

Note that you can also fax these documents to them using the number 800-455-7143. Also, VA instructs that you DO NOT send original documents, such as the veteran's discharge doc. Instead, copy them as these documents WILL NOT be returned to you.
